Parental Consent & Release Form

If you have more than one child participating, complete one form per child.

I am the parent/guardian of a minor, who desires to participate in the following school  activity: Quebec City Trip. Date of Activity: January 25-28, 2024.

I acknowledge that I have been fully informed as to the nature of the activity and the provisions for my child’s involvement and consent to my child’s participation in the above-described school activity. I understand that due to the nature of the activity my child will be in close proximity with students, staff, and other individuals, and that this activity may include an enhanced risk of illness, including the possibility of my child contracting COVID-19.

In consideration of the permission granted to my child to participate in the above-described activity by Pinkerton Academy, I release and hold harmless Pinkerton Academy, its agents, employees, and officers from any and all actions or causes of action of any nature for personal injury or property damage of any kind arising in any way from my child’s participation in the above-described school activity. I further acknowledge that this release is binding upon my heirs, successors and assigns: that I have read the foregoing and understand its significance and that I have executed this document voluntarily.

Health Information Sheet

This form will be provided to the doctor or medical personnel to whom your child is taken in the event of a medical emergency while on a school-sponsored field trip. Please complete ALL sections as accurately as possible and return to your field trip organizer.

Code of Conduct for Overnight Trips

  1. NO drug consumption of any kind (except prescribed medications as prescribed by doctor, with parental approval and knowledge of the chaperones).*
  2. NO alcohol consumption of any kind.*
  3. NO breaking curfew or failing to communicate whereabouts, as set by chaperones.
  4. Respect quiet hours, and respect property (of hotel & fellow travelers).
  5. NO hanging out of windows (travel vehicle or hotel).
  6. NO one is allowed in your sleeping quarters (only your assigned roommates).
  7. NO riding on private or public transportation without a chaperone’s consent.
  8. All students will remain in assigned groups for safety at all times, as indicated by chaperones in each context.  No exceptions. 
  9. Listen to, and follow instructions of chaperones and guides.
  10. You must be on time at designated meeting times and locations.  

*Breaking rules 1 or 2 can result in you being sent home early, at your expense.  Additionally, misconduct during school activities is reported to administration. Should you receive disciplinary action (detention, suspension or expulsion), it could impact your student status and potentially jeopardize your ability to graduate.

I understand the above rules and understand the consequences. I will abide by these rules while on this trip.   I recognize that this is a Pinkerton Academy sponsored trip.

I understand that the above rules, if broken while on the school trip, may cause my child’s immediate expulsion from this trip and subsequent disciplinary action.  I have discussed this Code of Conduct with him/her and s/he has promised to abide by it.  In addition, I may have retrieve my child or pay travel expenses for his/her early return.
